Below are links to supplementary forms which may be required with your on-line application process. The checklist provided during the on-line application process will guide you to which forms you will need.

Authorization and Release This form is a required document which must be submitted with your application fee. *REQUIRED*	Fee Schedule This form lists all fees and filing deadlines as set by the board.
Complete Written Instructions for On-line filing These instructions are provided DURING the on-line filing process as well. When you begin filing on-line, simply click the "Print Help" option at the top menu to view these same instructions.	Sample Application under Rule 302 This application is a sample application printed during the on-line filing process. It is provided as reference material for all applicants filing under Rule 302.
List of Criminal History Record Repositories This is a list of criminal record repositories to assist in the collection of the required criminal history records.	List of Driving Record Repositories This is a list of driving record repositories to assist in the collection of the required driving history records.
Personal Verification of Driving Record Information This form is used when you need to verify that you do not have a driving record in a particular jurisdiction. Read the form for details.	Personal Verification of Criminal History Information This form is used to verify that you do not have a criminal record in a particular jurisdiction. Read the form for details.
Employer Statement This form must be completed by an applicant's current employer to verify compliance with Pa.B.A.R. 302.	List of Certificate of Good Standing Repositories This is a listing of contact information to assist in the collection of the required certificates of good standing (used to show proof of practice in another jurisdiction).
Name and/or Change of Address Form This form is to be filled out and faxed or mailed to the Board Office to notify us of any changes.	Attorney Disclaimer Form This form should be used by applicants licensed to practice in another jurisdiction that are currently working in a legal capacity in PA.
Amendment to Application For Post Submission of On-Line Application This form should be used to update your application after it is submitted.
